I am sorry if this is a double post.

If I catch a pull out of bounds ( and I mean 3 steps at least OB), is it a turnover?

No, catching a pull is never a turnover. Dropping a pull (or otherwise touching it before it hits the ground, after which it does hit the ground), whether in bounds or out, it a turnover.

Is there an obligation to call "middle" before the pull is caught out of bounds?
This assumes the receiver wants middle.

Otherwise, I believe the catcher can immediately walk to the sideline, self-check the disc, and start play.

VIII.B.7 says "If the pull is caught, the disc is put into play at the spot on the playing field nearest to where it was caught." VIII.B.6.d talks about your options if the disc initially hits an out-of-bounds area, and is the only thing that mentions "middle". So, it seems that if you want to take it to the middle, you have to let it hit the ground. I expect that this is in place to keep people from going OOB to catch discs that were going to come back in and taking them to the more advantageous middle starting point, and I would personally never call it on someone who catches something that's clearly only headed further away (down a hill, into long grass, towards rim-destroying pavement, etc.).
